联盟税表W-8BEN空表格下载及W-8BEN填写范本解释

如果我们有做国外联盟的朋友应该是知道的，需要我们填写非美国用户填写W-8BEN税表，有的是要求我们直接在后台提交，有的是需要提交给指定的邮箱账户，但是相似点就是表格都一样的，我们只要填写即可。当然如果有商家给予指定的W-8BEN税表的话，我们就直接在他们后台下载，如果没有的话，老蒋这里也有提供下载过来的范本。

我们可以看到下载的表格中，加深黄色的代表是必填注意和填写的。



这里我们一一来解释到底应该如何填写。

1、 Name of individual who is the beneficial owner ：姓名

2、 Country of citizenship：国家

3、 Permanent residence address (street, apt. or suite no., or rural route)：居住地址详细到门牌号

4、City or town, state or province. Include postal code where appropriate. 城市/省份/邮编

5、 U.S. taxpayer identification number (SSN or ITIN), if required (see instructions) ：美国税号，没有就填写N/A

6、Foreign tax identifying number (see instructions)：填写你所在国家的税号，没有就填写 N/A

7、 Reference number(s) (see instructions) ：填写我们所在联盟账号

8、Date of birth (MM-DD-YYYY) (see instructions)：生日，按照标准日月年。

9、 I certify that the beneficial owner is a resident of：后面填写国家。

所有的信息都要用英文或者拼音填写。最后我们可以看到需要签字的，需要打印下来手写签字，最后是签字的日期，需要按照日期格式的日月年。
